How do I know what services my loved one needs?

  • We are able to do a complimentary consultation, which will include reviewing all care needs. Based on over 20 years of experience and our preferred partnership knowledge, our Team will be able to help navigate if Independent Living, Assisted Living, Memory Care or other services would be beneficial.

How much are your services?

  • Our services are free to the public, including a consultation, scheduling tours, helping with necessary paperwork and steps needed to complete move in.

What documents are needed when moving to a community?

  • The documents that are preferred, but may vary by community are Living Will, DNR, Power of Attorney (Medical & Financial), Health Surrogate, Probate and Legal Guardianship. If you are considering a move to an Assisted Living & Memory Care Community, you will also need an 1823 Health Assessment Form, which our Team can assist you with, prior to your move.

How much does a community cost per month?

  • Communities will vary based on the type of community (independent, assisted or memory care), style of apartment, and care services needed. Our Team has a monthly cost comparison tool that helps many of our families understand what to expect with a move compared to current living costs. When our Team meets with you, we will only recommend communities that will fit your budget.

What should I look for in a community?

  • When we meet with you, we will review your top priorities and what is most important to you. Some important aspects to consider are the care provided, social aspects, dining accommodations, and safety.

How soon can I make a move for my loved one?

  • With our Team, we can typically navigate a move as soon as possible, based on community availability. We will work with you to ensure all documents are in place, assessments are completed and necessary steps are taken prior to move in.

Does my loved one need an assessment?

  • If you are considering assisted living or memory care, then their Team will conduct an assessment. As assessment is simply questions to understand all of their current care needs, including activities of daily living, medication management, and more.
